Between Providers? Why Bridge Care Keeps Your Health on Track

There are a lot of ways to end up without a provider who knows you. You moved. Your doctor retired. Your insurance changed. Your next primary care appointment is two months out, and your prescription runs out in two weeks. You just left the hospital and nobody has looked at your medication list. If any of that sounds familiar, you are in the gap I built this practice to fill.

What bridge care means

Bridge care is a provider who steps in between where you were and where you are going. It is not meant to replace a primary care provider or a specialist. It keeps your care moving safely while you wait for the next stage.

Who it is for

  • Patients new to Tennessee or new to the area who have not established care yet.

  • People between jobs or insurance plans who still need ongoing management.

  • Anyone whose primary care appointment is weeks or months away.

  • Patients just out of the hospital who need a medication review and a plan.

  • Travelers and travel nurses whose care has to move with them.

What I can help with

Medication refills for ongoing conditions, after I review your history and confirm the medication is appropriate. Follow-up for conditions you already have. A medication reconciliation after a hospital stay. Labs when they are indicated. Referrals when something needs a specialist. I do not prescribe controlled substances, and if something is outside my scope or needs an in-person exam, I will tell you directly and point you to the right place.

Why thoroughness matters here

A quick refill is easy to do carelessly. I review your full medication list, your history, and your recent labs, because a refill written without that review is how interactions and gaps slip through. That is the difference between keeping you on track and just keeping the prescription going.

What happens when you are ready to move on

When you land with a new primary care provider or a specialist, I send a clear written summary of what we have done so you are not starting over. Some patients stay with me for ongoing care, and others use this as a bridge. Either is fine.

What this means for you

You should not have to go without care because the system is slow. If you are between providers, new to the area, or waiting on an appointment, I would be glad to help you stay on track. You can learn more about ongoing care and services here, or book a visit to get started. I offer virtual visits from wherever you are in Tennessee.
