Welcome to Garrett Health Services, LLC

Below is a summary of our key policies and patient consents. Full documentation is provided securely in our patient portal and must be reviewed and signed before receiving care.

  • • We provide telehealth consultations, messaging-based care, urgent care visits, and wellness support within the scope of practice.

    • Telehealth allows convenient access from home but has limitations, including limited physical exams and reliance on video/audio quality.

    • Telehealth care is non-emergency. For urgent or life-threatening issues, call 911 or go to an emergency department.

    • Patients must be physically located in Tennessee for telehealth visits.

  • • Secure portal messaging is preferred and recommended for non-urgent communication.

    • Response time is typically 24–48 hours during business hours. Messaging is not for urgent issues.

    • Email or text communication may be used, but full privacy cannot be guaranteed. Texts may include reminders or treatment information; message/data rates may apply.

  • • Labs (blood, urine, saliva, hair, stool, or skin tests) may be recommended to support your care. Costs are the responsibility of the patient.

    • Prescriptions are provided at the discretion of the provider. Certain medications, including controlled substances, may require in-person evaluation or monitoring.

    • All patients are expected to use medications as prescribed and report any adverse effects.

  • • All services are cash-pay. We do not file insurance claims.

    • Payment is due at the time of booking for visits or memberships.

    • Missed appointments require 48-hour notice; otherwise a $50 fee applies.

    • Administrative requests (letters, forms, documentation) may incur a $25 fee or higher for complex requests.

    • Credit cards may be stored securely for recurring payments.

  • • We protect your personal health information (PHI) in compliance with HIPAA.

    • PHI may be used for treatment, payment, healthcare operations, appointment reminders, and as required by law.

    • Patients have rights to access, amend, or request restrictions on their health information.

    • Full HIPAA/privacy policy and patient rights are provided in the portal.

  • • You have the right to be treated with respect, receive information about your care, and participate in decision-making.

    • You are responsible for providing accurate information, following treatment instructions, attending appointments, and respecting clinic policies.

  • By using Garrett Health Services, you acknowledge that you have read and understand these policies.

Full consents and detailed policies are provided and signed through the patient portal before any care is delivered.
