Leaving the hospital is just the beginning of recovery. Many patients are discharged with new medications, follow-up instructions, and unanswered questions — but can't get in to see their provider for days or weeks.

A telehealth follow-up visit allows for review of your discharge instructions, medications, and symptoms from home — without waiting for an in-person appointment.

Based out of Cookeville, TN, we serve the Upper Cumberland region, as well as patients located across the state of Tennessee.

Hospital discharge follow-up visits are conducted as a Hospital Follow-Up Visit ($175). Visit may be adjusted to a Standard Care Visit ($150) if clinically appropriate.
