Most healthcare access assumes you have a home-state PCP, a stable insurance network, and enough time between assignments to establish care somewhere new. Travelers usually have none of that. Contract-based insurance can leave gaps in coverage. Even when coverage exists, it's often tied to a network you're nowhere near once you're on assignment. Finding a new provider every few months, just to get a refill or handle something minor, isn't realistic, so a lot of travelers just don't. Things get put off until they can't be anymore.

Medication refills lapse because the prescribing provider is three states and two contracts behind.
