Returning Patients — If you have been seen before and need a follow-up or refill.
New Patients — If you’ve never been seen before, and want to book a new patient visit. POTS evlauations and Weight Loss Consults are found here.
Ongoing Care Plans — Enrolling in a monthly care plan for the first time.
Before your visit - Complete your patient setup
You will receive an automated email with detailed instructions:
Register in the patient portal
Your confirmation email includes a link to create your account in our secure portal. There is also a app available if preferred.
Fill out your health history
Medical history, current medications, allergies, and any relevant background — completed at your own pace before your appointment.
Review & sign consent forms
All consents are handled digitally through the portal. No fax, no mail, no printer required.
